Of course, family members are always welcome to attend activities with residents at Stonebridge.
Social Services A licensed social worker on staff promotes family involvement and support. At Stonebridge, our goal is to know our family members as well as we know our residents. A family support group meets each month to offer information and encouragement. Regularly scheduled care plan meetings provide an opportunity for family members to actively monitor and participate in the care of their loved one. At Stonebridge, health-care providers firmly believe in the power of family involvement when it comes to the health of its residents.
Care Plan Meetings Residents and families are encouraged to attend interdisciplinary care plan meetings. These meetings facilitate interaction between the various medical-care departments and provide a formal means of communicating with one another about the care of each resident. At a minimum, care plan meetings are held quarterly or more frequently when a resident's care needs change.
Dining and Nutrition With an on-staff professional dietician and a dedicated culinary staff, Stonebridge meets the specialized nutritional needs and therapeutic diets of each resident by serving three delicious meals a day. As part of its commitment to delivering the finest care to its residents, the staff dietician consults monthly with residents to ensure all nutritional needs are being met.
